For child or adolescent patients ages 5 to 17 years old who need to establish care for psychiatric medication management. Appointment length 90 to 120 minutes. Please complete new patient paperwork prior to appointment or arrive 20-30 minutes early to complete. Self-pay rate of $400 or copay will be collected at time of service. Available as telehealth.

For patients who need a brief follow-up for psychiatric medication management. Appointment length up to 30 minutes. Please complete follow-up patient paperwork prior to appointment or arrive 5-10 minutes early to complete. Self-pay rate of $150 or copay will be collected at time of service. Available as telehealth.

For patients who need extended follow-up time for psychiatric medication management. Appointment length 30 to 45 minutes. Please complete follow-up patient paperwork prior to appointment or arrive 15-20 minutes early to complete. Self-pay rate of $225 or copay will be collected at time of service. Available as telehealth.

For patients who need a telephone call of more than 5 minutes between appointments.  Prorated at $300 per hour.

For patients who need documentation completion of more than 5 minutes between appointments. Prorated at $300 per hour.

For patients who need prior authorization assistance beyond 10 minutes for medications between appointments. Prorated at $300 per hour.
